At Galesburg Cottage Hospital, we believe that being educated about diabetes is vital to successful self-management. People with diabetes must balance diet, exercise, medication and self-testing. The more you know, the more active you can be in your care. Our goal is to provide you with the tools needed to manage the disease, avoid complications, and maintain your mental and physical well-being.
Professional Staff
The Cottage Diabetes Education Program is staffed by registered nurses and dietitians. Staff members work together with you as a team to develop a treatment plan customized to your needs. They will enlist the support of your family and your primary care physician to make sure everyone involved in your care works toward common goals.
Individualized Education
Upon physician referral to the program, you will receive an individual consultation with a diabetes nurse educator and a registered dietitian to review your treatment plan and help you develop an individualized eating program. Staff members are also available to provide individual instruction about insulin pumps, gestational diabetes or other diabetes-related situations.
Group Sessions
In addition to individualized educations, patients participate in four two-hour group sessions. A blood test will be taken before and after completion of the classes to help you assess improvements in managing your blood sugar levels. Program topics include the following:
