All patients receiving care at Galesburg Cottage Hospital must register prior to treatment to receive either an inpatient or outpatient number. Patients will be given their patient care number either through the Patient Registration Department, located near the front entrance of the hospital, or through the Emergency Department. Patients should have the following information available at the time of registration:

• Patient's Social Security Number
• Insurance or medicare card, including the billing address, policy number, etc.
• Guarantor (the person responsible for paying the bill) information such as address, Social Security number, date of birth, etc.
• Power of Attorney/Living Will information
• Physician order (if coming from a doctor's office)
