Thank you for choosing Galesburg Cottage Hospital for your healthcare needs. The mission of Galesburg Cottage Hospital is to provide quality medical care in a cost effective manner. To fulfill our mission it is important that we follow sound financial policies. The following information will outline the policies that are followed in handling your hospital bill. Payment Responsibility

Galesburg Cottage Hospital will accept cash, check, money order, Visa and Master Card. All insurance deductibles and co-insurance portions are also to be paid in full at the time of service. Financial assistance is available to assist patients who demonstrate a financial need. If you are unable to pay your hospital bill in full and need to set up a payment arrangement or need financial assistance please contact our Financial Counselor at 309-345-4586. Commercial Insurance

As a courtesy to you, the Patient Financial Services staff will bill your insurance. In order for us to file your insurance you must provide the complete name, address and policy number of your insurance. All insurance claims will be allowed up to 45 days from the billing date to make payment on your bill. If insurance payment is not received within 45 days, the bill will become your responsibility. Prior Authorization or Precertification

Most insurance companies require prior notification for all inpatient and some outpatient services. Prior Authorization or Precertification is your responsibility. Please follow the instructions of your insurance company. Failure to meet your insurance requirements may result in a denial or penalty.

Medicare

The Patient Financial Services staff will bill Medicare for you. Before we can bill Medicare you will need to provide your Medicare number and sign the required Medicare forms. Additionally, if you are covered by a secondary or supplemental insurance, we will file your Medicare deductible and/or coinsurance portions to your insurance. If payment is not made by your secondary or supplemental insurance in a timely manner, we will request payment from you. Medicare or supplemental insurance may not cover some procedures your physician orders. Please see Payment Responsibility section.

Accident and injuries

A hospital bill incurred as the result of a non-work related accident, injury or other cause involving a third party, will be considered your responsibility. Litigation resulting from these accidents may take months or even years. If your insurance company will not make payment until the litigation is resolved, you will be personally responsible for the balance.
